Registration forms are available by calling 802-254-8026, writing the camp office, or by downloading the form as a Microsoft Word document or as an Adobe Acrobat PDF.

In addition, insurance and the government require that a Health History Form and a Vermont Family Size and Income Form be submitted.  Click here to download the Health History Form as a Word document or a PDF.

Print them on white or light-colored paper, fill them out and include them with your registration and payment check.
